Occupational Therapy at Home

There is a gap between what someone can do and what they want to be able to do, and that gap is exactly what occupational therapy addresses. Whether it's getting back into the kitchen after a stroke, managing a shower safely with arthritis, or finding ways to stay engaged with life despite cognitive change, an OT finds practical solutions that work in the real world.

What is Occupational Therapy?

Home-based occupational therapy involves a qualified OT assessing the client's functional abilities in the context of their actual home and daily life. The OT identifies what is difficult, what is unsafe, and what adaptations, equipment, technique or environment, would make the biggest difference. Recommendations may include adaptive equipment, home modification planning, cognitive strategies for people with dementia, or direct retraining of daily living tasks.

OT is valuable for anyone whose ability to perform daily activities has been affected by age, illness, injury or surgery, people post-stroke, those with arthritis or joint replacement, individuals with dementia, people recovering from surgery, or older adults in general wanting to maintain independence.

How it works

Initial assessment

The practitioner conducts a comprehensive in-home assessment, understanding your goals, current function and the specific challenges in your home environment.

Treatment or management plan

A tailored plan is developed with you, what we're working on, how we'll measure progress and how often we'll visit.

Regular sessions

Scheduled visits for treatment or management, with exercises or strategies for between visits where appropriate.

Goal review and progress

Progress is reviewed regularly. When goals are achieved, we plan for maintaining gains independently or transitioning to a maintenance programme.

🩺 Clinical note: OT assessment is often a prerequisite for approving home modification funding under Support at Home. Our OTs understand the documentation requirements and produce assessments that meet funding approval standards.
