Wisteria's dementia support is provided by carers with specialist training who understand the condition and bring consistency, routine and warmth to every visit.
What is Dementia & Cognitive Support?
Dementia support involves trained carers providing tailored assistance that accounts for the cognitive, behavioural and functional effects of dementia, helping with daily living in ways that work with the person's remaining capabilities, managing behavioural symptoms humanely, maintaining meaningful routines, and supporting communication as language becomes more difficult.

Who benefits from this service?
Dementia support is for people in any stage of Alzheimer's disease, vascular dementia, Lewy body dementia, frontotemporal dementia or other cognitive conditions. Support is calibrated to the stage of dementia and is also available for family carers themselves.
How it works
RN-led assessment
Our Registered Nurse conducts a thorough assessment including cognitive status, functional abilities, behavioural patterns and family situation.
Dementia-specific care plan
A detailed plan is developed accounting for the person's specific presentation, preserved strengths, triggers and communication preferences.
Specialist carer matching
We assign carers with specific dementia training and, where possible, those who share cultural or language background.
Regular clinical review
Dementia is progressive. Our RN reviews the care plan regularly and adjusts proactively, not reactively.
🩺 Clinical note: Dementia care at Wisteria is clinically supervised by our RN, who monitors for complications including infection, delirium, falls, pressure injuries and malnutrition, all of which present atypically in dementia.
