What is Medication Management?

Medication Management at home involves qualified nurses providing skilled, evidence-based care according to an individually developed care plan developed in consultation with our clinical team.

What's included

Medication administration (oral, topical, inhaled)
Webster pack preparation and management
Injection administration (insulin, anticoagulants)
Medication reminders for scheduled doses
Coordination with GP and pharmacy for script management
Support worker medication prompting and reminders
Prescriber communication and liaison
Blister pack coordination with pharmacy

Client and family education

Who benefits from this service?

Medication support is appropriate for clients who need help remembering to take their medications at the right times. For clients with complex medication needs, our care team can coordinate with your GP, pharmacist and community nurse. Support workers can provide medication reminders, while nursing staff can assist with more complex medication-related needs where appropriate and within scope.

How it works

1

Clinical assessment

Our RN conducts a thorough assessment and develops a detailed, individual clinical care plan.

2

Schedule and coordination

We establish a visit schedule matched to clinical need and coordinate with your GP and specialist team.

3

Skilled nursing care

Regular nurse visits deliver the clinical care, fully documented and communicated to your medical team.

4

Ongoing review and escalation

Clinical status is monitored at every visit. Concerns are escalated to your GP promptly, not at the next scheduled appointment.

🩺 Note: When clients are managing multiple medications, our care team monitors for any changes in routine or presentation and communicates promptly with the GP. We work within community nursing scope and always defer complex medication decisions to the prescribing doctor or pharmacist.
