Home modifications support involves a qualified occupational therapist assessing the home environment, identifying what changes would improve safety and independence, preparing documentation for funding approval, and coordinating with builders or tradespeople to have the work completed. How it works
Initial assessment
The practitioner conducts a comprehensive in-home assessment, understanding your goals, current function and the specific challenges in your home environment.
Treatment or management plan
A tailored plan is developed with you, what we're working on, how we'll measure progress and how often we'll visit.
Regular sessions
Scheduled visits for treatment or management, with exercises or strategies for between visits where appropriate.
Goal review and progress
Progress is reviewed regularly. When goals are achieved, we plan for maintaining gains independently or transitioning to a maintenance programme.
