When a client needs supervision through the night, requires assistance getting to the bathroom, is at significant falls risk in the dark, or is managing a condition that can deteriorate without warning, Wisteria provides overnight and around-the-clock care at home. Families can sleep knowing someone trained and present is watching over their loved one.
What is 24-Hour & Overnight Care?
24-hour and overnight care involves a Wisteria carer being present in the home through the night. Overnight care typically involves an active carer who provides assistance as needed. Extended or continuous care involves carer rotation to ensure someone is always awake and available. All overnight arrangements are coordinated by our RN.
What's included
Who benefits from this service?
Overnight and continuous care is appropriate for people with high falls risk at night, those with dementia who wander nocturnally, clients recovering from surgery, those requiring night-time catheter care, clients on palliative care pathways, and anyone whose family can no longer safely provide overnight support.
How it works
Clinical assessment
Our RN assesses overnight risks and determines the appropriate level, active overnight, sleep-over with call capability, or continuous care rotation.
Carer introduction
Overnight carers meet the client before the first overnight stay. Familiarity matters enormously at night.
Night care delivery
Carers follow a documented night care plan, check-in times, required interventions, escalation protocols and emergency contacts.
Morning handover
A full handover is provided to family or day care team each morning.
🩺 Clinical note: Falls in the night are a leading cause of serious injury in older people and significantly more dangerous than daytime falls due to delayed detection. Our overnight carers are trained in falls response and emergency escalation.
