What is Allied Health Coordination?

Allied health coordination is provided by Wisteria's care coordinator as part of every care plan involving more than one allied health discipline. It covers identifying which services are needed and why, securing referrals, scheduling appointments, sharing assessment findings across providers, ensuring recommendations are actually implemented in the daily care plan, and keeping the family informed throughout. The goal is a care team that functions as a team.

How it works

Initial assessment

The practitioner conducts a comprehensive in-home assessment, understanding your goals, current function and the specific challenges in your home environment.

Treatment or management plan

A tailored plan is developed with you, what we're working on, how we'll measure progress and how often we'll visit.

Regular sessions

Scheduled visits for treatment or management, with exercises or strategies for between visits where appropriate.

Goal review and progress

Progress is reviewed regularly. When goals are achieved, we plan for maintaining gains independently or transitioning to a maintenance programme.

🩺 Clinical note: Allied health assessment without implementation is common and represents wasted clinical investment. Our coordination service ensures what the physio, OT, dietitian and speech pathologist recommend is actually translated into day-to-day care delivery.
